Do I need to order one or is this item a set of two for the hot and cold ?

The Moen 115001 is the whole kit, it will come with both the hot and cold side.

how do I know if I need this kit. I am installing a granite countertop, maximum of 1-1/4 inches thick. I am installing Moen faucet T6420 but it does not have a note specifying the dimension of the countetop thickness that would require this kit.

The Moen 115001 is needed when the deck is greater than 1.25 inches thick. The Moen T6420 can be installed on decks up to 1.25 inches thick without the thick deck kit.

We want to purchase the 6121BN kingsley faucet but we will install 1 1/4 in granite. What kit do we need to order?

The Moen 115001 is only intended for a widespread valve body, and not a centerset. I am not able to find an extention kit for use with this centerset faucet.

The Moen 115001 is used only if you have a deck thickness of more than 1.5 inches.

Does the extension kit also come with an extension for the spout as well as the hot & cold?

The Moen 115001 includes an extension for both hot and cold valves and the faucet spout. It is designed to be used with the Mone 9000 widespread valve.
