The diverter button on the 04231 will allow you to switch back and forth between the shower head and the hand shower.
The Hansgrohe 27164 a system that is intended to upgrade your shower and they are implying that the upgrade is "as easy as 1-2-3." Yes, this does come with a shower arm.
You should read the Installation Sheet as it provides detailed instructions. But in the meantime, You need to purchase a Hansgrohe 27458 wall elbow supply to supply water to the handshower. Then to use the showerhead, push the handshower up into the handshower holder on the bottom of the showerpipe. Any Hansgrohe thermostatic/volume control valve will work, but check out the Hansgrohe 32317 Single Handle ThermoBalance II Thermostatic Valve trim (valve required and can be added during checkout) which works great with the style of the handshower.
The Hansgrohe 27164 is compatible with any Hansgrohe trim. You will need a trim that has a diverter however so you can utilize the handshower.
The Hansgrohe 27164 could be plumbed to work with the Hansgrohe 13241, but I would check with your plumber first to see it would work for your set up.
The Hansgrohe 27164 will need a trim (Hansgrohe 04231) to control the water, temperature and diverter for the option of turning off the hand shower. The 04231 has all of this in one trim. The rough in valve (Hansgrohe 01850) works with the trim behind the wall to direct traffic, so to speak.
The Hansgrohe 27164 will need a ThermoBalance to adjust the water tempurature and perhaps a volume control to govern the water flow.
