I want to be able to use my shower head and hand held shower at the same time or one at a time. Do I need a two way or three way valve?

In order to do this, you will need a three way diverter. Port 1 for shower, Port 2 for handshower, Port 3 for both.

Does this valve allow two shower heads to be on at the same time and one on and one off?

The American Standard R430 is designed to port water in 1 of 3 directions and is not used with combining multiple directions of flow.

Does this valve work with trim kit T005.430

The American Standard T005.430 takes the R540 or the R520 for the rough in.

My diverter is too far into the wall, is there an extension kit?

American Standard does make an extension kit which is available here in a few different finishes.

Perhaps the person that suggested a three way diverter for shower head and handheld shower can answer this question: how would you recommend plumbing this? If I plumb, A, A+B, and B to each of the three outlets, when I turn on A, water will go through the AB connection and go to B as well. I didn't think about this, until I turned the water on. My thought now is to use two check valves in order to keep water from going from A or B through AB, but this wasn't mentioned in the post. Is there another way?

The American Standard R430 is a 3 way so it will only do 3 separate devices or 2 with a single and combined operation. I hope that helps.
